First, create a shop owner on the island and give them the bathhouse interior obtained from the wishing fountain, then start building the hot spring exterior. Make the main gate using the house exterior option with a triangular roof, then give that exterior to the owner so their house becomes the hot spring hotel gate. Make the small buildings on both sides with triangular-roof objects, build walls with cube objects, make shoji screens with upright board objects to separate the baths, then landscape the area with pools, paths, flowers, trees, and the finished screens.
